We know your time is valuable so we offer three convenient options of filling out the necessary new patient paperwork.

1) You may arrive 15 minutes prior to your scheduled appointment time to fill out the forms in our office.
2) You may download and fill out the forms below on your computer, print and bring with you on the day of your appointment.
3) Download and fill them out on your computer, save them to your computer and then either e-mail or fax back to us. Our fax # is 336-791-0935 and our e-mail is businessmanager@smilesofcarolina.com
